Use this form to request a calculation to see the maximum additional amount you
may contribute to the Thrift Plan. Make-up contributions are in addition to your
current contributions and are applied to the after-tax portion of your account;
they do not receive an employer match.
Use this form to contribute make-up contributions, after you have had a make-up
calculation of the maximum additional amount you may contribute to the Thrift Plan.
If you need to request a calculation, use the
Thrift Plan Make-up Calculation Request form.
Use this form to make loan payments in addition to those taken from your paycheck.
You can also use it to make delinquent loan payments and to make regular monthly
loan payments if you are no longer working for a Participating Employer.
Use this form if you want to give consent for someone else, such as your spouse,
to have access to information about your Thrift Plan savings account.
Terminated/Retired Employee with Outstanding Loan
For Loans Started Prior to December 1, 2004
Use this form to choose your option in regards to your outstanding loans after terminating/retiring
from employment with a participating employer.
Do not use this form if you started your loan December 1, 2004, or later.
For Loans Started December 1, 2004 and Later
Use this form to choose your option in regards to your outstanding loans after terminating/retiring
from employment with a participating employer.
Do not use this form if you started your loan prior to December 1, 2004.
The Thrift Plan qualifies as a Safe Harbor 401(k) Plan under the Internal Revenue
Code and annually provides a safe harbor notice to on-going participants as well
as new hires. The notice contains information on participant’s rights and obligations
under the plan.
