Find a Medical Provider in the
First Health National PPO Network

Selecting the link below will open a new browser window and take you to the Web site of First Health. DMBA has contracted with First Health to provide the physician network outside Utah, Southeast Idaho, Hawaii, and Washington.

Once on the First Health Web site, select "Electronic Directory - Medical". You must close the First Health browser window to return to DMBA's Web site .

If you experience any problems accessing the First Health Web site, you may call 1-800-237-5702 to locate a First Health provider.

You can also nominate a doctor for inclusion in the First Health network by completing First Health's nomination form. (Adobe's Acrobat Reader is required. If needed, you can download it for free)
