Mental Health Provider Information

Deseret Mutual is currently not contracting with licensed mental health providers, i.e. LCSW, ED.D., Ph.D., LPC, or MFT.

However, you may fax a resume to us at 801-578-5916 and we will keep the resume on file .  If we have need for a particular specialty we will review the submitted resumes, and contact the provider with an application to complete.
